1. 申請成績單請提供百分制成績資料。
2. 延長收件期間：自即日起至115年6月9日（星期二）下午5時止。
3. 請備妥相關申請資料，並以親送或郵寄方式送達中醫學院辦公室。
4. 郵寄申請者，請於信封封面註明「申請○○○○○○獎學金」。
